Tri-Heart Plus (ivermectin/pyrantel) chewable tablets are for use in dogs 6 weeks and older to prevent canine heartworm disease and for the treatment and control of roundworms and hookworms. Tri-Heart Plus contains ivermectin/pyrantel, the same proven active ingredient for heartworm protection and internal parasite control as the leading competitor, but at a lower price. A simple and palatable monthly, chewable tablet protects by killing the larvae before they reach a dog's heart. Beef flavor.
Each chewable tablet contains: Up to 25 lbs - 68 mcg ivermectin and 57 mg pyrantel; 26 to 50 lbs - 136 mcg ivermectin and 114 mg pyrantel; 51 to 100 lbs - 272 mcg ivermectin and 227 mg pyrantel.
Directions: Tri-Heart® Plus ivermectin/pyrantel chewable tablets should be administered orally at monthly intervals at the recommended minimum dose level of 6 mcg of ivermectin per kilogram (2.72 mcg/lb) and 5 mg of pyrantel (as pamoate salt) per kg (2.27 mg/lb) of body weight

How to Order Prescriptions from PBS Animal Health
We fulfill livestock, equine, and pet prescriptions through our Matthews Veterinary Pharmacy. To process your prescription order, we need a valid prescription on file. We can receive a prescription through the three methods below.
Our pharmacy service is available in these states: CT, FL, IA, IL, IN, KY, MI, MN, NH, NY, OH, PA, TN, WI and WV.
1. Place your order online and our pharmacist will contact your veterinarian.
During the checkout process, we will collect your veterinarian and animal information. Our pharmacist will contact your veterinarian to obtain a prescription. We will ship your non-prescription items immediately. Prescription orders will be held up to 3 business days awaiting approval from your vet.
2. Ask your Veterinarian to submit your prescription by one of the following methods.

- Fax the prescription to our Pharmacy at 330-830-2764
3. If you have the original prescription from your vet, please call our Customer Care team for ordering instructions.
